Abstract

Background: Recurrent falls cause disability and dependence among older adults. Conventional orthostatic assessment may overlook delayed systolic blood pressure recovery during the first minute after standing.

Objective: To determine whether delayed systolic blood pressure recovery after active standing is associated with recurrent falls among community-dwelling older adults.

Methods: This prospective cohort study was conducted at Continental Medical College, Lahore, Punjab, Pakistan, from February 2024 to February 2026. One hundred adults aged 65 years or older were recruited between February 2024 and February 2025 and followed for 12 months. After 10 minutes of supine rest, blood pressure was measured at baseline and at 30 seconds, 60 seconds, and three minutes after active standing. Delayed recovery was defined a priori as systolic blood pressure remaining at least 10 mmHg below the supine baseline at 60 seconds. Falls were recorded prospectively using monthly fall calendars and telephone interviews. Recurrent falls were defined as two or more falls during follow-up. Adjusted risk ratios were estimated using modified Poisson regression with robust standard errors.

Results: Delayed recovery was identified in 34 participants. Recurrent falls occurred in 15 of 34 participants with delayed recovery and 8 of 66 with normal recovery (44.1% versus 12.1%). Delayed recovery remained independently associated with recurrent falls after multivariable adjustment (adjusted risk ratio 2.73; 95% confidence interval 1.29–5.77; p=0.009).

Conclusion: Delayed systolic blood pressure recovery at 60 seconds was independently associated with recurrent falls. Early postural blood pressure measurements may complement conventional orthostatic assessment and improve fall-risk identification in older adults.
